What does this error usually look like?
The button shows up and changes color when you hover over it, but clicking it changes nothing on the screen. No message appears.

For the AI, drawing a button and programming what it does are two separate tasks. Sometimes it does the first and forgets the second, especially when your request had many things in it at once.
Another common cause is a button that depends on something that doesn’t exist yet, such as a database that hasn’t been set up. In that case the action starts and fails without telling you.
How do you avoid it next time?
When you ask for a new button, also say what should happen after the click and what the person should see on the screen. For example, “when I click Save, the order appears at the top of the list and the form is empty again”.

Tell the AI that, and say when it works and when it doesn't. For example, "it works when I fill in everything, but not when I leave the phone number empty". That detail often points straight to the cause.
